Faulting module libCtrlV318.dll

Hi,
In a recent installation of WinCC OA 3.18, we have many application crashes and errors logged in the Windows event viewer.
The faulting module is reported to be libCtrlV318.dll.
Sometimes it does not occur for nearly 3 weeks, then it happens multiple times a day.

The impact is pretty problematic as the Desktop UI is just gone without any indication and the control room is no longer hearing alarms - they don't always have the screen in sight, so the application silently crashing is a big problem.
They are not yet asking to sound an alarm every 5 minutes just so they know the UI is still alive, but that might not take long before they do...

Has anybody seen similar behavior or knows how to investigate?
